New York State Civil Service Law entitles all district employees to take up to 4 hours of paid leave annually, without charge to leave credits, for cancer screening. The screening includes physical exams specifically for cancer screening detection. Travel time is included in the 4-hour cap. Absence beyond the 4 hours must be charged to personal time. The leave is not cumulative and expires at the close of business of the last day of each fiscal year.

District employees must complete the below form and return the form to the Office of Human Resources after their physician has completed the medical provider section of the form.
