MEDICAL LEAVE INSTRUCTION PROGRAM

If a student has a medical condition requiring temporary tutoring instruction, a note from their physician stating their diagnosis, any medical restrictions, and the duration of the restriction (no more than a 6 week increment at a time) should be submitted to the school nurse.  If it is a mental health related request, there is a form the physician must complete and it is sent to Donna Fowler in the Office of Student Support Services.  Tutoring will be arranged by the Office of Student Support Services with Washington Irving.
